There
are two ways to submit your entry.
1. Online entries: Log on to visit
www.faeriefilms.com/foodforthought/ between 12:01
a.m. PT on 7/6/05 and 11:59 p.m. PT on 12/31/05 (new
extended date as of 11/1/05) and follow on-screen
instructions to register, complete a quick survey,
and submit your 250-words or less story explaining
how you personally have been affected by food, nutrition
or exercise at school. All online entrants must have
a valid e-mail address. In case of dispute as to identity
of the winner who enters online, entry will be declared
made by the registered user (meeting eligibility criteria)
of the e-mail account, and if a prize is won, it will
be awarded to that registered user,
2. Mail in entries: Send in your
clearly printed or typed 250-words or less story explaining
how you personally have been affected by food, nutrition
or exercise at your school. Include your name, address,
and home telephone phone number on the top of all
materials submitted and mail to: Faerie Films "Food
for Thought” 23412 Pacific Park Drive, Aliso
Viejo, California 92656. Mail-in entries must be postmarked
by and Contest ends 12/31/05 (new extended
date as of 11/1/05).
Entrants may enter more than once but each entry must
be unique and follow the requirements listed or entry
will be disqualified. Faerie Films is not responsible
for lost, stolen, illegible, misdirected, postage
due or late mail submissions. All entries become the
property of Faerie Films.
Entries must meet all of the following requirements
or they will be disqualified:

Stories must not exceed 250 words
-
Each entry must be original, unpublished, the sole
property of submitter, and not previously submitted
in any other contest and may not infringe upon any
third party rights
-
Each entry must include: entrant's full name, street
address, city, state, zip code and home phone number.
Online entries must include e-mail address

At the judges' discretion, entries including unsuitable
content or that are in violation of third party
rights (including, but not limited to, copyrights)
will be immediately disqualified
Prizes:
Five (5) Grand Prize winners will receive $500 cash.
Five (5) runners-up will each receive movie gift certificates
worth $100.
Odds of winning depend upon the number of eligible
entries received. Faerie Films reserves the right
to substitute prize of equal or greater value. Federal,
state and local taxes are the sole responsibility
of the winner. Total award value for all prizes is
$3,000.
Judging:
Judges will select the winners from all eligible entries
received. Entries will be judged on insightfulness,
drama, and overall impact. Winners will be notified
by phone, postal mail and/or e-mail on or about 1/15/06
(new extended date as of 11/1/05).
By
entering the Contest, entrants accept and agree to
these rules and the decision of the judges, which
shall be final. Parent or legal guardian will be required
to sign a Winner's Agreement and Release on behalf
of the minor, and return it by date specified in notification
letter or prize will be forfeited in its entirety
and another winner may be selected. Return of any
prize or notification as undeliverable will result
in disqualification and an alternate winner may be
selected.
By
submitting an entry, entrant agrees to allow use of
his or her name and entry for advertising and promoting
this or similar promotions, including posting of their
first name and last initial on the website, without
compensation unless prohibited. Entrant also agrees
and acknowledges that Faerie Films may edit and publish/post
the entry prior to use, and that the entry is entrant's
original work, and that Faerie Films’ use will
not violate any third party's rights. Faerie Films
reserves the right to use all stories submitted by
the five (5) Grand Prize and five (5) Runner Up winners
as material for any future film project or documentary.
All entries and all rights of ownership and publication
of same will become the property of Sponsor and will
not be returned.
Other
Conditions: Sponsor assumes no responsibility
for computer system, phone line, hardware, software
or program malfunctions or other errors, failures
of any kinds, delayed computer transmissions or network
connections that are human or technical in nature.
Sponsor reserves the right in its sole discretion
to cancel or suspend this Contest, in its entirety
or in part should virus, bug, tampering, unauthorized
intervention, fraud, technical failures, or other
causes beyond Sponsor's control corrupt the administration,
security, fairness, integrity or proper play of the
Contest. Sponsor is not responsible for incorrect
or inaccurate entry information whether caused by
Internet users or by any of the equipment or programming
associated with or utilized in the Contest by any
technical or human errors which may occur in the processing
of the entries in the Contest. Sponsor reserves the
right to withdraw the transmission of this Contest
at any time and award the Prize from among eligible
entries received prior to the date of the withdrawal.
Sponsor reserves the right at its sole discretion
to disqualify any individual it finds to be tampering
with the entry process or the operation of the contest
or website or to be acting in violation of the Rules.
Eligibility: Contest is open to legal
residents of the 50 United States and The District
of Columbia, 10 to 18 years of age, except employees
and the families of Faerie Films, each of their affiliates,
subsidiaries, promotions and advertising agencies.
Faerie Films, and its affiliated companies, representatives,
agents or employees are not responsible for, and shall
be indemnified by the prize winner against, any claims,
injuries, losses or damages of any kind resulting
from acceptance, use, misuse, possession, or loss
of the prize. Void where prohibited or restricted
by law. Subject to all federal, state and local laws.
Winner's
List: For a copy of the winner's list, send

92656. Requests must be received by January 31, 2006.
